The following information must be provided before a student may be registered:
Proof of Residency (i.e. lease/purchase agreement, utility bill with recent post mark, notarized letter from landlord, letter from city clerk's office)
Immunization records- student will not start until nurse has verified records to be up-to-date
Identification of person registering student. Person must have legal custody of student.
IEP or 504 Plan (if applicable)
Are you transferring your student to another academic program?
Important Message: You must call the Guidance Office immediately to fill out the proper paperwork. Without a formal withdrawal, Franklin High School cannot forward necessary records to another school. We are here to help you make the transition as smooth as possible. Failure to properly withdraw your child may also create problems when he/she becomes an adult and needs to trace academic records for employment or another educational insitution.
Parents: It is very important that we have a way to reach you either directly or through another person in the event of a medical or other emergency. Forms will be provided at the beginning of the year but please contact us immediately if your contact information or address changes during the year.
TO REQUEST A TRANSCRIPT
Fill out a form in the Guidance Office and give to Mrs. Johnson.
Send a written request to Cynthia Johnson, Registrar, Franklin High School, 115 Central Street, Franklin NH 03235, or email her at: email@example.com
Include either the year of graduation or, if you withdrew before graduating, your last year of attendance.
Indicate your name at time of attendance (if different from your current name), your current address and phone number.
Enclose a check (made out to Franklin High School) for $3 for each transcript requested. Transcripts will not be mailed until payment is received.
State the address to which you want the transcript(s) sent.
Official transcripts can be mailed directly to the college you are applying to or to you if you prefer. They are mailed in an envelope with a seal across the flap. If the transcript is mailed to you, do not open the envelope if you are going to present it to a college as it will no longer be official. Transcripts are generally mailed within 24 hours of receiving payment.
